• Do not use the Registration Form if you are applying for a stipend, use the Stipend Application Form.
• Please submit one (1) form per person attending workshop sessions.
• Direct Support Staff must register separately, but may apply for stipends.
• Please PRINT or TYPE clearly – Include complete information.
• Meals ARE INCLUDED in your registration fees EXCEPT for Thursday Lunch, and Friday Breakfast.
• The Awards Banquet is a special event and IS NOT INCLUDED in the Registration Fees.  Tickets must be purchased separately and cost $40.

• NO REFUNDS AFTER MAY 21, 2010.

• All requests for special accommodations must be received no later than May 7, 2010.

• Hotel reservations must be made by each attendee NO LATER THAN May 10, 2010.
• Hotel lodging costs $99.00/night + 10.5% tax for a total of $109.39/night.
• Hotel reservations can be made by calling 1-800-937-6660 and you must mention the MegaConference to receive the conference rate.
